A collaboration skills score form is a tool you can use to evaluate students’ skills in working in groups. The form lists skills important for effective collaboration, such as communication, conflict resolution, and taking responsibility. You can use the form to assess student performance in group work and give directions for improvement. You can also use a scoring form for peer feedback.

A collaborative skills scoresheet includes for example:

  • A list of skills such as communication, conflict resolution, and taking responsibility.
  • The assessment criteria you use to evaluate student performance.
  • Rating scales used to assess student performance. For example, these scales can range from unsatisfactory to excellent, or from 1 to 5.
  • A comments section in which you make specific comments about your students’ performance. For example, to give pointers for improvement or to highlight specific positive aspects.
  • Room for improvement – a section in which students can include their own areas for improvement.

A variation of this score form is the peer feedback form where group members assess each other and give feedback. FeedbackFruits Group Member Evaluation allows students to anonymously give each other feedback on collaboration skills, for example, where as a teacher you can also include their peer judgement to raise or lower the final grade of an assignment.
