A professional conduct score form is a tool you can use to evaluate your students’ professional behaviour to prepare them for their later work. In this form, you include criteria appropriate to the subject matter, such as their commitment, responsibility, communication skills, and work attitude. 

You can use this form for any project or assignment, helping your students to learn and grow in their professional skills. You can also ask students to assess themselves and compare this assessment with yours. This will make them more aware of their own reflective abilities. You can also ask their fellow students or any colleagues at an internship to fill in this form. Feedback on professional behaviour is often part of a final assessment, but is not used as the only factor. 

A professional conduct score form can be used, for example, for medical students doing internships. Using the form, you assess them on the following items, among others: decency and respect, communication with patients, dealing with patients’ emotions, dealing with nurses and colleagues, handling their own emotions, norms and values, self-reflection and dealing with feedback, commitment and involvement and responsibility (integrity, reliability, knowing one’s own limits). Where the different components can be scored on different scales.
